New Student Requirements for Domestic and 国际学生 

国内学生:

入职前:  New students born after 1956, who have been accepted by the university 必须 submit all three immunization information and records outlined below, through the 学生健康门户网站:    http://studenthealth.basilinfracon.com  (Login with your J Number and JagMail Password)

1. Documented proof of two doses of a measles containing vaccine (rubeola, MR, MMR). One dose 必须 have been after 1980 and at least one of the doses 必须 have been an MMR. You may provide documented immunity to the measles, mumps, and rubella in lieu 疫苗.
2. All new incoming students 必须 complete the Tuberculosis Screening Questionnaire on 门户网站.  Further testing may be necessary.  
3. All students living on campus 必须 provide proof of having received the Meningitis (MenACWY) vaccine within the last 5 years by uploading to 门户网站. MenACWY vaccines include Menactra, Menveo, and MenQuadfi. MenB vaccines (Bexsero and Trumenba) will 不 be accepted in place of MenACWY vaccines.

Go to the Forms tab and upload a copy of your records under Immunization Entry Form
